This form may be used by Optimist Clubs and junior golfers to register to play in the tournaments selected from the list in Step 1.

STEP3 — TERMS & AGREEMENT

Please Note: Entry fee must be received by mail to be officially registered.

Any Contract arising upon acceptance of this application by Optimist Clubs shall be deemed to have been made under and governed by the laws of the Province of Ontario and it is agreed that this application and the web site, of which this application forms a part contain all terms and conditions agreed upon by the parties, and no other agreements relating to the subject matter of this agreement shall be deemed to exist. By checking the box below you agree personally and on behalf of the applicant and their respective personal legal representative that the maximum liability of Maximum Potential and its servants and agents, on any claims whatsoever, whether in contract, tort (including negligence) or otherwise, for any loss or damages to the Applicant or to any other person arising out of this contract, shall be limited to the amount paid to Optimist Clubs hereunder, and they shall indemnify and save harmless Optimist Clubs for any and all claims, damages or expenses, including solicitor and client costs, suffered in connection with this contract. And agree to the use of any photographs take during these events.
